Donor Letter of Intent for Gift

To initiate a Donor Letter of Intent to Give, please complete and submit the form below. Upon submission, you will receive an email containing a copy of your completed Letter of Intent. You may then download and print the document for delivery to your Financial Advisor, who will facilitate the transfer of the gifted securities.

All information required by the Foundation to complete the transfer process will be automatically transmitted to Pamela Mills for processing.
