Donor Letter of Intent for Gift

To initiate a Donor Letter of Intent to Give, please complete and submit the form below. Upon submission, you will receive an email containing a copy of your completed Letter of Intent. You may then download and print the document for delivery to your Financial Advisor, who will facilitate the transfer of the gifted securities.

All information required by the Foundation to complete the transfer process will be automatically transmitted to Trust Services Department for processing.

Please Note: The email delivery can take up to 15 minutes. If you do not receive the form email, please check your SPAM folder.

Shares of DTC transferable securities should be delivered from your (the donor’s) investment account to Georgia Baptist Foundation’s brokerage account at:

The Foundation will provide you with a contribution acknowledgement substantiating the date shares settle in the Foundation account. The transfer of securities listed on the following page(s) is an irrevocable charitable donation to the Georgia Baptist Foundation for the benefit of your chosen Christian ministry.
